Minor Library Card Request

Flat Rock Public Library offers parents or legal guardians of minors (under the age of 18) to submit applications online to obtain a library card. Library card applications will be processed and library cards mailed out within one week. Patrons who already have a library account can renew their library card online. Patron seeking to replace a library card will be charged $2.00, which will be due upon receipt of the  replacement card. By submitting this form you agree to the charge.
Library card applications for minors require the parent or legal guardian (guardian with court papers) to act as a “sponsor.” As sponsor, patrons agree to accept responsibility for all materials borrowed on the card including all fines incurred and lost or damaged materials. Parent, Guardians and minor patrons also agree to abide by the rules and policies of the lending library.

Photo ID showing proof of residency in Flat Rock, Gibraltar, or Rockwood is required to validate your address. If the minor does not live in Flat Rock, Gibraltar, or Rockwood, the minor can apply for a Student Library Card. You will need to upload a document proving enrollment, such as a school form or school ID.
